The Community Paramedicine Integration Pilot (C.P.I.P) Program is an innovative healthcare model aimed at delivering essential medical support beyond the walls of hospitals and clinics. Focused on vulnerable and underserved populations, the program provides doorstep healthcare services delivered by trained community paramedics. By bridging the gap between primary care and emergency services, the CPIP Pilot Program helps reduce unnecessary emergency room visits, improves long-term health outcomes, and empowers individuals to manage their health safely at home.
Whether it's routine wellness checks, chronic disease management, or education on healthy living, the program ensures patients receive personalized, accessible, and proactive care.
To be eligible for the C.P.I.P Pilot Program, individuals must meet one or more of the following criteria:
(e.g., diabetes, hypertension, heart disease, asthma, CHF, COPD )
Especially who have difficulty accessing healthcare
Physicians, community health workers, hospitals, clinics
That would benefit from community health interventions
Outreach programs and health screening initiatives
Community paramedics providing doorstep healthcare services
Nutrition, hygiene, and disease prevention
Blood pressure, Blood glucose levels, Oxygen saturation, Lung auscultation
Connecting patients to higher-level medical facilities when needed
