C.P.I.P's Proposed PILOT PROGRAM

Welcome to C.P.I.P Pilot Program!

The Community Paramedicine Integration Pilot (C.P.I.P) Program is an innovative healthcare model aimed at delivering essential medical support beyond the walls of hospitals and clinics. Focused on vulnerable and underserved populations, the program provides doorstep healthcare services delivered by trained community paramedics. By bridging the gap between primary care and emergency services, the CPIP Pilot Program helps reduce unnecessary emergency room visits, improves long-term health outcomes, and empowers individuals to manage their health safely at home.

Whether it's routine wellness checks, chronic disease management, or education on healthy living, the program ensures patients receive personalized, accessible, and proactive care. 


 Please note: This is a proposed program outline and is not currently active. CPIP is in the process of building partnerships and gathering support to bring this initiative to life. 

Eligibility Criteria for C.P.I.P Pilot Program

Referral-Based Criteria

→ Patients referred by healthcare providers

By doctors, family physicians, midwives clinics, hospitals, or  emergency departments.

→ Individuals identified through C.P.I.P initiatives

Outreach programs and health screening initiatives 


Intake criteria

To be eligible for the C.P.I.P Pilot Program, individuals must meet one or more of the following criteria:

1. Individuals with chronic illnesses that require ongoing monitoring

(e.g., diabetes, hypertension, heart disease,  asthma, CHF, COPD  )

2. Recent discharge from hospital, & require follow-up care

Physicians, community health workers, hospitals, clinics

3. Elderly with limited mobility

  Especially who have difficulty accessing healthcare 

4. Individuals with disabilities who require home-based healthcare service

5. Individuals experiencing social isolation

That would benefit from community health interventions 


Services Provided by C.P.I.P

Community paramedics provide doorstep healthcare services

→ Health assessments & wellness checks

→ Public Health Education

Nutrition, hygiene, and disease prevention

→ Chronic disease monitoring & management

→ Assistance with mobility & fall prevention for elderly individuals

→ Vitals monitoring

  Blood pressure, Blood glucose levels, Oxygen saturation, Lung auscultation

→ First aid and basic emergency response training for community members

→ Coordination with hospitals, specialists, and local health workers

 Connecting patients to higher-level medical facilities when needed 

→ Medication management & education
